The Bible Is Our Standard

the Pastor and church leaders in making improvements in the leadership

roles to provide effective and competent spiritual guidance to new members. 

Please complete the following survey as it applies to you as a new member. 

Please make sure you completely answer all questions according to the

 instructions given for each one.  Use black ink to complete the survey.
